We are so excited to welcome you and your family to The Chapel on a Sunday morning! Filling out this form in advance will expedite the check-in process on Sunday morning. Be sure to fill this out as parent/guardian first, then click the "+ Add child" button for each child (birth through 6th grade) that will be participating in our programming on Sunday. See you Sunday!
